Complete your Questionnaire/Form on Client Portal
Your questionnaire helps your firm gather the key facts it needs to move your case forward quickly and accurately. Completing it early reduces back‑and‑forth, prevents delays, and unlocks the next steps in your Workflow.
Tip: After filling in all answers in the required fields, it is important to click on Submit for Review button to mark the task as completed.
Who this is for: Clients filling out a questionnaire in the Glade client portal.
Steps
Sign in to your Glade client portal and navigate to the form request task from Home or Activity section.
Click the Form Request task
When the questionnaire opens, fill out the form.
Fields labeled Optional can be skipped.
Fields without the “Optional” label are required and must be completed before you can submit.
When you’re done, review your answers and click Submit Questionnaire.
Result: Your task is marked Completed in your workflow, and your firm is notified for review.
Tips
If you can’t submit, scroll back up—any missing required fields will need to be completed.
Use clear, complete answers to avoid follow‑ups.
Unsure about a question? Message your firm for guidance through the discussion chat in Activity section.
Troubleshooting
Don’t see “Complete Questionnaire” on Home? Refresh the page. If it’s still missing, check the Activity view or contact your firm.
Submission won’t go through: Look for any required fields you missed; they’ll prevent submission until completed.
